This notice describes how your health information may be used and shared, and your rights over it. We are a mobile optician service: we dispense and fit eyeglasses at your home, workplace, or other location you choose. We do not perform eye examinations of any kind, and we do not bill insurance — all services are self-pay.
Information we may collect
Your name and contact details, the prescription you provide, your prescriber's name and practice, visit details (address, entry notes, contact preference), and payment information for your order.
How we use and share it, without your separate written authorization
- To provide your care: prepare for your visit, fit and dispense your glasses, and send your prescription and lens specifications to our wholesale lens laboratory so your glasses can be made.
- To verify your prescription: we contact your prescriber only if you give us permission to do so.
- To run our service: schedule and remind you about visits, process your payment, and keep the records the law requires.
We do not sell your health information and do not use it for marketing without your written authorization. Any other use or sharing requires your written authorization, which you may revoke in writing.
Your rights
To see and get a copy of your health information; to ask us to correct it; to ask for limits on certain uses or sharing; to ask us to contact you in a particular way or at a particular place; to get a list of certain disclosures we have made; and to get a paper copy of this notice at any time. Complaints
If you believe your privacy rights have been violated, you may complain to us at the contact above or to the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
Changes
We may change this notice, and the changed notice will apply to all health information we hold. The current notice will be posted on wiseoldowlopticalcompany.com.
